REMINDER:
Your child's Universal Child Health Record is needed annually. This is a form required of every licensed center in NJ; it must be filled out by your child's pediatrician; and must not exceed 12 months from the date of the exam. Please ask your pediatric office to fax the completed form to us at: 888-889-4132. No cover page is needed. The form can be found here.
Additionally, we require the most recent copy of your child's Immunization Report indicative of your child having received all age-appropriate vaccines. Among them must be a flu vaccine received on or after September 1st, but not later than October 31st, for each school year. There's no form for this; we simply need the report provided by your pediatric office. It, too, can be faxed to us at 888-889-4132.
